Dear brother *****, as a servant of the Lord I lay my hands upon your head and give you a Patriarchal Blessing according to your desire. Dear brother, you have been richly blessed of the Lord. You were faithful and valiant in the Spirit World. Your spirit was active and you were known by your Father in Heaven, and He kept you back that you might come to earth in the day when the Gospel was restored in its fullness and come to a good home that you might not partake of the sins of the world. Thou art of the Royal House of Israel through the loins of Ephraim, a chosen and choice son in Israel. At your birth the Lord gave you a great mission to perform in life. If you will be faithful and true to the covenants you have made in the waters of baptism and when you received the Holy Melchizedek Priesthood, you will be richly blessed. You shall have the spirit of discernment and the Lord will be mindful of you. He will help you through the whisperings of the Holy Ghost so that you will be able to fulfill the great mission that was given unto you. Keep yourself sweet and clean, unspotted from the sins of the world.

Keep the Word of Wisdom, pay your tithes and your offerings, and the Lord will magnify you before the people. You will be a great leader in the wards and stakes of Zion where you will reside. You will receive great knowledge, even hidden treasures will be revealed unto you and your testimony will grow and increase. You will be called to many responsible positions in the Church.

The Lord has given you a life's companion worthy to be the mother of your children. The day will come when you will take her to the House of the Lord and be sealed for time and all eternity. You shall have a good home and you will be a good provider. You will find joy and happiness in your children. As you labor in the organization of the Church, many will come to you for counsel and advice. It shall be your privilege to preach the Gospel to many people. You will be able to instill into their hearts a knowledge of the truth and a testimony of the Restored Gospel. You shall see many things take place in these the latter days. Much trouble and sorrow await the world. If you will be true, you shall be spared and the Angels of Death who are sent to earth now to destroy wickedness shall spare you from much sorrow and sickness and disease. You shall have the spirit of discernment. If you will seek for the companionship of the Holy Ghost you shall be spared from much trouble and sorrow. Dear brother, the Lord has been merciful unto you. You have been chosen in these last days to come to Zion to the stakes of Zion where you can advance in the Church and help bring souls unto Christ and build up Zion. What a glorious privilege that has come to you to come to Zion in these the latter days to the shadows of the Temple where you can rear your family. You will find joy and happiness in your posterity.

Dear brother you shall see many wonderful and glorious things transpire as you visit the sick. You shall have the power of healing and you shall see the power of God manifested in blessing the people over whom you shall preside. The day will come when you will be chosen and called to stand before the judgment bar of God and if faithful to the end you will be welcomed home and you will hear, 'Thou good and faithful servant, enter into the joy of the Lord.' This is eternal life, to be with them throughout all eternity. Dear brother, seek for these things for these are your blessings. The Lord will bless you and you will be able to fulfill the great mission you have been called to in honor and in glory to your Heavenly Father. If you will be kind to those less fortunate you shall not want for the good things of life and your basket shall be filled. I seal you up to come forth on the morning of the first resurrection, the resurrection of the just, to take your place at the head of your posterity and they shall arise and call you blessed, for you have work to do for many of your kindred in the House of the Lord. When you go and do the Temple work for them you become a saviour upon Mt. Zion for your kindred dead. These blessings I seal upon you by virtue and authority of the Holy Priesthood and in the name of the Lord Jesus Christ, Amen.

The Lord is mindful of you and He knows you because of your faithfulness and your devotion to His work in the Spirit World, and He gave to you a wonderful mission when you were born into this earth.

Thou art of the House of Joseph, through the loins of Ephraim, a chosen daughter of Israel. Because of your sweetness and your devotion to the Lord, He sent you to earth that you might come to a good home and be taught the gospel of Jesus Christ and enjoy all the blessings the Church has in store for the Children of God. He gave you a great mission and that is motherhood, and you will receive some very choice spirits to your home.

In the due time of the Lord He will give you a life's companion, one holding the Holy Melchizedek Priesthood. He will take you to the Temple of the Lord. You will be sealed for time and all eternity. Along with that you will have great responsibility of teaching and training these little souls and become a co‑worker with your Heavenly Father in preparing these little bodies. He sends the spirits from the throne of God. You shall have a good provider and a lovely home, and the Lord will shower His blessings upon you and you will be made happy.

You will enjoy your work in the different organizations in the Church and will become a leader among your associates, and they will seek you for counsel and advice.

Pray always, and the Lord will answer your prayers. Keep yourself sweet and clean, unspotted from the sins of the world. Pay your tithes and your offerings, keep the Sabbath day holy, keep the Word of Wisdom, live the laws of nature and the Lord will shower.

His blessings upon you and in time of need He will remember you. In time of sickness and trouble, the Angel of death will pass your door by and you will be spared from much sorrow and trouble.

You shall be tempted and you shall be tried. The evil one will try to lead you astray. Pray always and the Lord will hear your prayers and when you are tempted, the Holy Ghost will guard and protect you and the evil one will have no power over you.

Honor your father and your mother that thy days may be long upon the land which the Lord has given thee, and you shall have a rich life and an abundant life.

You shall be called to fill a mission for the Church and this will be a rich life for you. You will enjoy it and as you lead souls into the water of baptism, with one holding the Priesthood performing the ordinance, you shall rejoice and feel the nearness of God in laboring and bringing souls unto Christ. Your tongue shall be loosed and you shall be able to preach in plainness and power and you will see many souls converted to the Church.

You shall be called to the Temple to labor as an ordinance worker. Search for your kindred dead for there are many, many wonderful, honest souls who have already accepted the gospel and are waiting for some loved one to do the vicarious work for them.

The Lord has blessed you with talents. Seek for wisdom and knowledge that you might even find hidden knowledge that it might be useful to you in fulfilling your great mission that was given to you.

Let your companions be those who have a testimony of the Gospel and it will help you, and will guard and protect you. You will find out in going through your life that the time to begin to serve the Lord is in your youth, when you can begin to understand and realize the doctrines of Christ and what is necessary to become exalted in the Celestial Kingdom of God.

Make your calling and election sure by keeping the commandments of God and you will be called where God the Father and the Son are, and be with them throughout all eternity. This is eternal life and it is your according to your desires, and everything that your heart desires shall be given you upon your faithfulness and devotion to the Church.

I seal you up to come forth on the morning of the first resurrection, clothed in robes of glory to take your place at the head of your posterity, and a multitude will arise and call you blessed.
