Dear Brother *****: According to your desire, and by the authority which I hold as a Patriarch in the Church of Jesus Christ of Latter Day Saints, I place my hands upon your head and give you a blessing and if you are faithful to the laws and the covenants of our Father in Heaven, the same shall be fulfilled in your behalf. You were greatly favored of the Lord in the Spirit World to be associated with your elder brother Jesus the Christ in the Grand Council that was held in Heaven before this world was created, and in that Grand Council you were ordained and set apart to come here upon this earth to labor in the ministry for the salvation of the human race. You were also one of the loyal spirits who gave thanks of gratitude to our Heavenly Father for coming here upon this earth and shouted with joy when Jesus Christ said let me go down upon the earth and I will suffer death for the salvation of the human race and I will give you the Honor and the Glory. You have been born of goodly parents in this goodly land, a land that has been dedicated and set apart for the restoration of the true and everlasting Gospel. You have also had the privilege in being united to one of the daughters of Zion, and many fine children have been born to you; and some of your seed shall be present and meet the Saviour when he shall again return upon this earth.

You are now about to go upon a Journey to the place were the Gospel had its beginning for the restoration in these the latter days, and in the name of the Lord Jesus Christ I bless you that you and your family shall arrive in peace and safety. You shall be successful in your labors. You shall be blessed with many of the good things of this earth. You shall have plenty for your self and family and some to spare for the poor and the needy. If you are faithful to the laws and commandments of the Lord and of this great and glorious Gospel you shall have health and strength to do your work; and I say unto you, at all times be prayerful and humble and if you are in doubt ask of Him who giveth to all men liberally, for He has made this promise to all who seek Him in prayer, and He will answer you.

You are of the lineage of Ephraim and you are entitled to the blessings of Abraham, Isaac and Jacob.

Dear Brother *****, I seal you up to eternal life. You shall come forth upon the morning of the first resurrection with all of your loved ones and many of your kindred, and I seal these blessings upon you conditioned to your faithfulness to the Gospel of Christ, and in the name of Jesus Christ.
