Dear Brother ***** in the name of Jesus Christ and by virtue of the Holy Melchizedek Priesthood in me vested as a Patriarch in the _____ Stake of Zion I place my hands upon your head and give unto you a Patriarchal Blessing. This is done at your request and upon the recommendation of your Bishop. It is given in the spirit of love and in all humility and it is hoped that it will aid you in living your life more fully before the Lord. You are numbered among the fortunate of the earth in that it has been your happy lot to be born in this day and age of the world when the gospel of our Lord and Savior, Jesus Christ is here in its fullness and you have availed yourself of the opportunity of becoming a member of this great organization and have lived to enjoy the blessings it offers those who faithfully serve the Lord. You have been born of worthy parents who have had an interest in your welfare and have pointed out to you the path that leads to life eternal. You hold the higher Priesthood in the church and have proven yourself valiant in the service of the Lord in that you have complied with his teachings and have lived to share in the fruits of the gospel. You have lived to see your children respond to your instructions and grow in the favor of our Heavenly Father because of your worthy example. For this you should be profoundly grateful.

You were valiant in the Spirit World where you supported the Savior and fought diligently by His side against Lucifer. You are of the lineage of Joseph who was sold into Egypt through the line of his son Ephraim. You are therefore of the choice seed of the earth; an heir to the blessings pronounced upon these great prophets of the Lord. As a member of the royal household of Israel you should count yourself most fortunate for this heritage and so live that you may receive the blessings in store for you. Born in this great land, America where you enjoy religious freedom and have profited by its free school system you are numbered with the choice of the earth. Here you have enjoyed the companionship of your lovely wife and have complied with the Lord's Commandments to multiply and replenish the earth. Sealed in His Holy Temple you are in line for an exaltation in His Kingdom if you keep the faith and do His work. Nothing greater can be hoped for or realized. You are admonished to continue steadfast to His cause. I seal upon you the blessings of health that you may live long upon the earth to do good and that your strength may hold out until your earthly mission is fully completed. I bless you in your basket and in your store that you may always have those things essential to your daily needs. I bless you with knowledge and pray that you may always have wisdom to make proper application of the things you learn.

I bless you with the power of discernment that you may always be in a position to solve your problems and to thwart the evil designs of those who would do you harm. I bless you with the spirit of love and pray that you will shower it upon your fellowmen and to continue to measure it out generously to your lovely family. I bless you with charity that you will continue to share with the Lord and administer to the wants of the needy. I bless you with the spirit of kindness and with a desire to spread the truths of the gospel that others may enjoy the same blessings that have come to you. May you always have the desire to council with the Lord in all you do and may you live long to enjoy the fruits of the land and the love of your family and fellowmen. Be faithful and your reward will be an exaltation in the Kingdom of God.
