*****, in the name of Jesus Christ, and by authority of the Holy Melchizedek Priesthood which I bear as Patriarch of the _____ Stake and at your request, I lay my hands upon your head and give you your Patriarchal Blessing, to be a guide and comfort to you throughout your life. The realization of these blessings depend upon your worthiness to receive them.

You are of the lineage of Ephraim, who was the son of Joseph, who received those promises which were made unto Abraham by the Lord, that through him and through his posterity all the nations of the earth would be blessed. I seal your inheritance blessings upon you, that you might go forth to receive all the blessings and promises which were made unto you before the foundation of the world might be realized. You are a choice spirit, and hands were laid upon your head and you were set apart, and ordained to come forth in the last dispensation, to be born of the House of Israel, and of the royal lineage, that you might bear the high and holy priesthood that you might be of the Elect of God, a servant of your Father in Heaven, a servant to your fellow men, that you might be an instrument in the hands of the Lord in helping to accomplish his holy purposes here upon the earth, and that through obedience to the laws and ordinances of the gospel you might fulfill the measure of your creation here upon the earth, and have joy in this life, and joy in your posterity throughout all ages of time and throughout all eternity. Great shall be your joy in your labors.

You are endowed by the Spirit with the Gift of Healing. This is a special gift which comes to you through the Holy Ghost, that you might accomplish your mission, that the powers of the priesthood might be manifest through you, that they upon whom you lay your hands in this holy ordinance might be raised from their beds of affliction, even that they might be raised from death to a continuation of life. Live always worthy of it. You are blessed with the health and strength necessary to accomplish all the Lord requires of you, and to carry out all the duties and responsibilities that are yours as a husband and father. You are blessed that you will enjoy the bounties of the earth, and the blessings of heaven that you and your family will not want, that you will have the means to provide for your family, and the time and means to accomplish the Lord's work.

Pray unto your Father in heaven for the strength to overcome the forces of evil which surround you in the world, for there are those about you who would entice you to partake of those things which would tend to destroy the spirit. If you will seek the guidance of the Lord in your daily life and for the welfare of your family, you will have the strength to overcome the adversary that you might walk uprightly before the Lord. You will be a leader among your brethren in the church and will be an instrument in the hands of the Lord in helping to bring to pass the promise made unto the house of Israel that they would receive their blessings at the hands of Ephraim, for you will truly minister in righteousness unto the lost tribes of Israel. Great shall be your joy in your labors. Trust in the arm of the Lord for you are his servant and he will watch over you and protect you throughout all the days of your life, and in the time of despair will lift you up and strengthen you that you might accomplish those things which are the righteous desires of your heart.

Seek after the information pertaining to those of your ancestors who have gone before you that the ordinances of salvation might be accomplished for them, that they might go forth and rejoice in the day of their salvation. If you will do this prayerfully and diligently, the way will be opened before you and all the information necessary will be made available to you. Go into the Temple often that these ordinances might be accomplished. You will be permitted to go into the Temple of the Lord and be Sealed with your family for time and for all eternity, which is the promise of exaltation in the Celestial Kingdom. Be faithful to those covenants which you make in the house of the Lord for they are covenants which only you can break. Read the scriptures, ponder them in your heart prayerfully and diligently and your mind will be opened that the mysteries of the gospel will be made plain to you that you might be prepared in all things to accomplish the work the Lord requires of you.

These blessings I seal upon you, together with all others which your Father in Heaven sees fit to bestow upon you according to your worthiness, and seal you up to come forth in the first resurrection, by authority of the holy Melchizedek Priesthood which I bear, and in the name of Jesus Christ; Amen.
