Brother *****: In accordance to thy desire and by virtue of the holy Melchizedek priesthood which I hold, I lay my hands upon thy head and as the Lord shall direct, give unto thee thy Patriarchal Blessing to be a benefit and guide unto thee throughout thy life in accordance to thy faithfulness. Thou art of the lineage of Ephraim the son of Joseph who was sold into Egypt and unto whom was given choice blessings and they shall be choice blessings unto thee. As thou shalt continue to grow in faithfulness and prove thy worthiness unto the Lord, thy testimony shall grow with thee and give unto thee the courage and strength to follow thy righteous convictions. And the Lord shall continue to be with thee to give unto thee counsel and advice and to give thee guidance and direction that thou shalt have opportunities and blessings given unto thee from time to time which shall be for thy good and that thou shalt have the opportunity of being of service in the work of the Lord. As thou shalt continue to keep thy habits in accordance with the Word of Wisdom and the laws of nature, thou shalt be blessed with health and strength of body and of mind and shall also receive the more important blessings promised, those of knowledge and wisdom, and as thou shalt seek knowledge and wisdom from the scriptures and from good books thy mind shall be receptive to true principles and thou shalt have a firm testimony of the gospel and an understanding of its principles and the plan of life and salvation.

It shall be thy privilege to further cultivate thy talents and receive a good education that thou shalt be better prepared to fulfill thy mission upon the earth and accomplish the duties required of thee from time to time. As thou shalt continue to magnify thy callings and honor the priesthood with its gifts and powers which have been given thee, thou shalt have the privilege of advancing in the priesthood and receiving of those higher obligations. And thou shalt rejoice in the powers of the priesthood which shall be manifest through thee for the good of thy household, thine associates and thy community.

As thou shalt continue to keep thyself clean and pure and virtuous and seek the blessings of the Lord in righteousness, even the blessings in the House of the Lord shall be granted unto thee for thou shalt have the privilege of going into the Temple of the Lord and having a choice companion sealed unto thee and shall rejoice in the blessings of fatherhood. Continue to have a desire in the heart to serve the Lord and keep His commandments and as thou shalt teach unto thy posterity the principles of the gospel and the plan of life and salvation, they shall bless thee for the inheritance which shall come unto them as as result of thy faithfulness and thy teaching. The opportunity shall be given unto thee to teach the gospel unto many and thou shalt rejoice in those experiences. Thou shalt be a savior upon Mt. Zion and shall rejoice in the opportunities of working for both the living and the dead, for thou shalt put forth thy efforts in accomplishing the work of the Lord required of thee, the Lord shall bless thee and give unto thee the abilities and the time and talents necessary to accomplish the duties required of thee. As thou shalt seek blessings of the Lord in humility and in righteousness and learn to heed the whisperings and promptings of the still small voice of thine guardian angel which shall be thy constant guide and companion, and shall be thy first impressions, thou shalt avoid many of the difficulties and trials in life, for thou shalt be warned in advance and instructed and guided and inspired to overcome and avoid those barriers and pitfalls in life. These blessings I seal upon thy head through thy faithfulness and seal thee up to come forth in the morning of the first resurrection, by the virtue of the priesthood I hold, and in the name of Jesus Christ, Amen.
